1) Redemption program of SmartHeart Dog Play Pen valid from 1 July 2019 – 30 September 2019.
2) Redemption program of SmartHeart Dog Play Pen is only valid with completed redemption form.
3) Purchase SmartHeart Dog Treat (Dog Biscuit) and SmartHeart Special Care Formula Product, collect original barcode(s) and attach with redemption form.
4) SmartHeart Dog Play Pen is only valid for redemption with the original barcode(s) of SmartHeart Special Care Formula 2.6 kg / 3.0 kg / 8.0 kg / 10.0 kg / 15.0 kg / 20.0 kg (Power Maxx, Power Pack for Adult & Puppy, Small Breed Formula, SmartHeart Nomal Puppy Food & SmartHeart Small Breed Puppy) and SmartHeart Dog Treat 100g (Dental Care, Shiny and Health Coat, Healthy Hip Happy Joint, Grilled Chicken, Grilled Beef, Fruit & Veggie).
5) Redemption form must include the combination of SmartHeart Dog Treat (Dog Biscuit) and SmartHeart Special Care Formula Product.
6) Submit the completed redemption form together with the original barcode(s) to the provided address.

7) Freebies will be delivered within 60 days after program ends, while stock last.

9) Perfect Companion (M) Sdn. Bhd. reserves the right to reject any invalid redemption form and barcode(s).
10) Perfect Companion (M) Sdn. Bhd. shall only accept form submission by post. Any form submission by hand shall not be entertained. Redemption prize shall be post via courier service only and any self-collection shall not be entertained.
11) Perfect Companion (M) Sdn. Bhd. expressly disclaims all liabilities arising from any lost or unsuccessful transmission of redemption form and prize. Proo f of submission of redemption entry is not proof of receipt, and the Organiser shall not be liable for any delay, lost, damaged and/or non-receipt of submissions.
12) Perfect Companion (M) Sdn. Bhd. shall only deliver prize to the valid and completed address as provided in redemption form.
13) This program is open to all residents in Malaysia, except for the employees of the Organiser and its group of companies with their immediate families & Organiser’s agencies associated with the program with their immediate families. Participants of this program must be 18 years old and above.
14) By submitting an entry to this program, Participants are deemed to have understood and agreed to be bound by the Terms and Conditions.
